Sample Reports At the Shaw Chiropractic Offices we take pride in the quality, thoroughness and accuracy of the reports provided by our physicians. We report information in an objective and unbiased fashion allowing for the reader to gain a complete understanding of issues surrounding the patient's status at the time of the evaluation. A report should paint a mental picture which is clinical enough to satisfy another physician yet understandable by a non-clinician.
